How to get your records

Salem Family Practice fulfills requests for release of medical records for all patients.

Our staff will be happy to assist you in obtaining copies of your medical records. Salem Family Practice Clinic procedures ensure that your personal information remains private, so you will need to provide a valid authorization.

You can fax, mail or personally deliver your authorization to release health information. 

We charge for copies if the records are requested for personal use or if the records are picked up at our office for use by a doctor, clinic, hospital or any other organization or business. The actual cost of copying records cannot be determined until a request is received and processed.

We cannot fax records to individual patients; patients who need records can pick them up at the front office.

The charge for copies of medical records is $5 for the first five pages and 5 cents per page after. 

There is no charge for obtaining copies of a patient’s medical records if the records are sent to a doctor’s office, clinic or hospital.
