To Sign Up: Contact our office and provide our staff with your email address. We will send you an invitation via email to sign up on the portal.

To Sign In: Click the patient portal button at the top of our website to log in. (Yellow Button)

New Patient

Information

Dr. McGarry requires all new patients fill out the new patient paperwork for her to review then once reviewed, we will call you to set up an appointment. This process takes about 1-2 days. You may pick up the paperwork at our office, request us to email it to you, or print it off our website. 

 

PHONE CALLS: 

 

Please allow up to 24 hours for someone to return your phone call. 

 

Usually, all phone calls are returned the same business day. Phone calls after 4:30 pm are sometimes returned the next day if we are busy. 

 

MISSED APPOINTMENTS:

 

If you are a new patient and miss your first visit, we will not reschedule.

 

Three (3) missed, no-show appointments will result in dismissal from the practice.

 

There will be a $25 charge for a missed appointment. We understand appointments sometimes need to be changed, so we ask that you please call our office in advance if you cannot keep your scheduled appointment.

 

 

LAB WORK:

For the convenience of the patients and the staff here at Salem Family Practice, we will call to provide the results ONLY if there is a problem.

Patients do have access to view all labs and tests done on your patient portal.

 

If you would like a copy of your lab results you may bring in a stamped self-addressed envelope with you to your appointment and once the results come in we will mail you the results OR you may stop in our office to pick up a copy at your convenience.

 

WE DO NOT ACCEPT LAB ORDERS FROM OTHER DOCTORS

 

 

MEDICATION REFILLS:

 

For refills on medications please call your pharmacy first. They will contact us directly.

 

MEDICATIONS AND PROGRAMS:

 

Salem Family Practice Clinic does NOT offer chronic pain management and will not prescribe chronic pain medications. For example, daily chronic narcotics or Xanax.

 

We will provide you with a referral to a pain management center if you need this specialized form of care after evaluation by Dr. McGarry.

 

Here at Salem Family Practice, we participate in the Arkansas Drug Monitoring Program. This program provides complete and detailed reports of all prescriptions that each and every patient receives at their pharmacy or pharmacies. The goal of this program is to enhance patient care and ensure legitimate use of controlled substances.

You must have your statement to get your "Form ID" and your account/chart number. 

If you do not have that information, give our office a call at 501-794-4110 and ask for Haley.
